Document your first aid assessment
After you complete your first aid assessment, you must document your findings. This includes the first aid supplies and services you’re required to provide.
You can use this first aid assessment worksheet or your own template.
Create and review your assessment document
Create an assessment document for each workplace. You must include information about:

The number of workers present
-
Barriers to first aid
- The time required to transport an injured worker to medical treatment, and the method of transportation
This document should also include:

The employer’s name
-
The workplace address or location.

The shift name or time, if applicable
-
The date of assessment
-
The joint committee members or worker representative consulted and the date of their consultation
Review your first aid assessment every 12 months or whenever something changes that could affect your first aid requirements.
Document all of your first aid assessments separately, even if there are no changes from the previous assessment.
