If you’ve been injured at work, we want to help you as soon as possible. In order to start your claim for services and benefits, we need you to report your injury to us so we can gather information about what happened.
We’ve recently improved our online injury reporting form to make it easier and more convenient to report your injury online.
The improved form is designed for workers with a physical injury from a single incident at work (i.e., an injury that did not develop gradually over a period of time).
What are the improvements?
Based on research and feedback from injured workers and our claims team, the online form improvements include:
- Ability to save your information: The form asks you to log in so that your information is saved automatically as you fill it out.
- Mobile-friendly design: You can complete the form on any device and access it 24 hours a day, 7 days a week.
- Easy to understand questions: The questions are simplified and relevant to your situation. The form walks you through each step and offers you the option to share more details if you choose.
- User-friendly navigation and visuals: You’ll see your progress as you move through the form sections and can use visuals to indicate which part of your body was injured.
