Submitting reports and invoices
Using our reporting and invoicing forms is the easiest way for you to get paid. Our fee schedule explains how we pay for dietitian services. You can check your payment status online.
Dietitian fee schedule
Our fee schedule sets out the rates we pay for your services. Please make sure you use our fee codes in your reporting and invoicing. Only services listed in the fee schedule can be covered.
Dietitian reports and invoices
To complete our required reports and to submit invoices, please use the following forms:
- For reporting, use Generic Report - Medical and Health Care (Form 83D51)
- For invoicing, use Generic Invoice - Medical and Health Care (Form 83D128)
- For pre-approved travel and expenses, use Provider Pre-Authorized Travel and Expense Confirmation (Form 83D36)
At the top of each form, you'll find instructions for completing and submitting it to us. Type your information and complete all required sections.
Please pay special attention to the reporting and invoicing codes. Using the correct codes will help us pay you sooner.
If there's a problem with your invoice, we'll send you a letter explaining why we are requesting corrections.
