Please be prepared to provide a reference number with your complaint. Valid reference numbers include: a claim number, employer number, prevention report number, prohibited action complaint file number, or review number.

Mail or fax
You also have the option to send your complaint in writing by mail or fax. However, we recommend using our online complaint form. You can still print the form and fax or mail it to us at:
