Worker fatally injured after bulldozer rolled down hog fuel pile

Incident summary
A worker was operating a bulldozer on the top of a pile of hog fuel (wood waste material). The bulldozer rolled down the face of the pile and overturned. The worker was partially ejected from the bulldozer cab and sustained fatal injuries.

Investigation conclusions

Cause

  • Bulldozer at outer edge of hog fuel pile rolled off pile. The worker was operating a bulldozer on the top of a pile of hog fuel. The bulldozer came too close to the outer edge of the pile, rolled down the face of the pile, and overturned.

Contributing factors

  • Worker not wearing seat belt. The worker was not wearing a seat belt and was not restrained in the cab of the bulldozer. As a result, when the bulldozer overturned, the worker was partially ejected out the rear window of the cab, which contributed to his injuries.
  • Reduced visibility. It is not known to what extent the heavy wind and rain at the time of the incident may have reduced the worker’s visibility. It is possible that he was not able to see the edge of the hog fuel pile.
