Fall from roof with too much slack in lifeline

Incident summary
A crew of four workers was removing metal decking from the second storey of a construction project approximately 6.7 metres (22 feet) above grade. The workers were wearing fall protection. When the workers had removed two rows of decking, one of the workers fell through an opening in the deck to the ground below. The worker suffered serious injuries.

Investigation conclusions
Cause

  • Worker fell without proper use of fall protection: After repositioning himself after a break, the worker did not adjust his rope grab to eliminate the slack in his lifeline. As a result, he had too much slack in his lifeline. When he fell approximately 6.7 metres (22 feet) to the ground, his fall protection equipment was not adjusted to either prevent or arrest his fall, and he sustained serious injuries.
