Generic Invoice — Medical and Health Care (Form 83D128)

The fastest and easiest way for health care providers to submit invoices for medical and health care is through our online portal, My Provider Services.

When you submit your invoice online you can save draft invoices, attach additional documents to your invoice, and see status and payment details.

Please note that to submit an invoice to us, you first need a payee number. If you don’t have one, you can request one here: Request a temporary payee number.

Download the PDF or Word form and mail or fax it to WorkSafeBC (this may result in longer processing times).
