Roofer seriously injured in fall from first-storey roof of house

Incident summary
Several roofers were working on a steep-slope residential roof. One roofer was working near the edge of the roof, moving a bundle of shingles, when he fell 3.4 m (11 ft. 3 in.) to a concrete driveway below. The roofer sustained serious injuries.

 

Investigation conclusions

Cause

  • Unprotected worker fell from roof. The roofer was not using any fall protection when he fell off the edge of the roof.

Contributing factors

  • Inadequate fall protection plan. The roofers were working on the roof without appropriate planning to ensure their safety at the time of the incident. The roofers and their supervisor were subjected to a fall hazard of 3 m (10 ft.) or more without an effective method of fall protection in place. Written fall protection plans were completed by both employers before the incident. However, the plans were not implemented prior to the incident, and it is unclear whether they were communicated to workers. Had an adequate fall protection plan been in place, this incident likely would not have occurred.
  • Inadequate supervision. The fall protection plan developed by the roofers’ employer had several deficiencies. In addition, the employer did not post the plan at the workplace, inform the workers about the hazards in the area where they were working, or ensure that they were using personal fall protection. The employer did not provide its workers with adequate supervision for the high-risk work activity that was being done at the time of the incident.
