Worker Prescription Claim (Form 3)

If you have prescription drug expenses related to your accepted claim, you can submit a request for reimbursement by logging in to your online services account.

Once logged in, select Reimbursements and then the Submit prescription expenses tile and follow the prompts.

When you submit your request online, you'll receive reimbursements faster and be able to track your previous submissions and reimbursements.
