You can request a review of an assessment penalty. Before submitting your request, please review Assessment Policy 5-261-1 to see circumstances in which a penalty may be reduced or cancelled.

The fastest way to submit a penalty review request is to log in to your online services account. Once logged in, select Manage my account under Insurance, then choose Request a review of an assessments penalty.
