Ground worker pulled into mobile wood chipper

Summary
A groundsman working as part of a tree-trimming crew was feeding wood debris into a driven-feed mobile chipper. He was inadvertently drawn into the chipper and sustained fatal injuries.

Conclusions

Findings as to causes

  • The driven-feed mobile chipper did not have a panic bar to act as an emergency stop device. The groundsman was unable to prevent himself from being pulled into the chipper blade.

Findings as to underlying factors

  • The groundsman was probably working or standing near the front of the feed table during chipping operations.
  • The groundsman's clothing (gloves or jacket) may have been caught by the branches being chipped.
