This form enables WorkSafeBC to coordinate radio frequencies on job sites. Employers operating tower cranes, self-erecting cranes, or industrial cranes need to submit the form:

• When cranes will be in operation for more than one week
• If there are any changes to the assigned radios
• Whenever the crane is moved to a different site
