Worker Supply and Services Claim (Form 3A)
If you have expenses for approved medical supplies and services on your accepted claim, you can submit a request for reimbursement by logging in to your online services account.
Once logged in, select Reimbursements and then the Submit health care or vocational services expenses tile and follow the prompts.
When you submit your request online, you'll receive reimbursements faster and be able to track your previous submissions and reimbursements.
Other ways to submit:
- Download the PDF or Word form and mail or fax it to WorkSafeBC (this may result in longer processing times). You can also view the guide.
