Application to Add or Change Personal Coverage for an Active Account (Form 1801)
If you already have an active account with WorkSafeBC to cover your workers, are self-employed as a partner or a proprietor, and want to purchase coverage for yourself, you can use this form to apply to add Personal Coverage to your account. You can also use this form to make changes to your existing Personal Coverage.
If you’re reactivating or setting up a new account with WorkSafeBC, please submit an Application for Registration and Personal Coverage for Proprietors and Partnerships (Form 180C).
This form can be printed and completed by hand, or it can be downloaded and completed digitally in Adobe Acrobat. If you don’t already have Acrobat on your computer, you can download Adobe Acrobat Reader, a free app, from get.adobe.com/reader.
