This report may be used in the following scenarios:

1. Requested by the WorkSafeBC officer. This report may be requested for an interim update on the worker's treatment or return-to-work (RTW) status. The report is due within 3 business days of the date of such a request.
2. Initiated by the provider. This report will be submitted by the there is a substantial update or change to the treatment plan.
