Application for Registration and Personal Coverage for Proprietors and Partnerships (Form 180C)

If you’re self-employed as a partner or a proprietor and you employ workers, you must apply to register with us.

You can also apply for Personal Coverage for yourself — whether you employ workers or not. Personal Coverage is workplace disability insurance available to sole proprietors and partners in a partnership, who aren’t automatically covered by the Workers Compensation Act. This insurance will pay health care, wage-loss, and rehabilitation benefits if you are injured at work.

You can easily apply online — it is quick and convenient. When you apply online, you can save your application in progress and return to complete it later, check your application status, and receive a confirmation number by email once you’ve submitted it.
