Employers are encouraged to complete and submit this form to WorkSafeBC's Radio Frequency Coordinator, to coordinate radio frequency on the job site.

Employers who use radios in forestry need to submit the form:

• When a machine is moved more than 5 kilometres from a coordinated location
• If there are any changes to the assigned radios
• If the operation stops for an extended period of time or when the radios are no longer in use
• On an annual basis as a frequency confirmation
