Apply for compensation and report a work-related injury or occupational disease using one of the options below. You’ll need details of the incident that caused your injury, your earnings information if you missed work, as well health care provider information if you sought medical treatment.
If you’ve missed work as a result of your injury, start a claim by calling Teleclaim at 1 888 WORKERS (1 888 967-5377), Monday to Friday from 8 a.m. to 4 p.m. The claims representative will also give you a user ID and password so you can go online later to check your claim details.
If you have a user ID and password, you can submit your Form 6 online and then log on later to see more details about your claim, including correspondence, claim decisions, and payment information.

Use this option if you have a health care only claim and have not missed work as a result of your injury. You’ll be able to submit your Form 6 online in a single session without setting up an online account.
