Incident summary
Two health care workers were travelling to a community health clinic when a loaded logging truck collided with the vehicle they were provided with by their employer. The collision occurred on a curve in the highway during poor winter driving conditions. The health care worker who was operating the vehicle sustained fatal injuries, while the passenger sustained serious injuries. The logging truck driver was not injured.

Investigation conclusions

Cause

  • Icy roads caused loaded logging truck to cross the centre line and collide with workers’ vehicle. The roadway was extremely icy, causing the logging truck to cross the centre line when it entered a corner on the roadway and collide with the workers’ vehicle.

Contributing factors

  • Driving too fast for road conditions. Driving too fast on an ice-covered road surface at a curve in the roadway contributed to the truck losing control, crossing the centre line, and colliding with the health care workers’ car.
  • Lack of anti-slip or de-icing materials during icy road conditions.The roadway had not been sanded or salted during icy road conditions. The addition of these anti-slip materials would have helped reduce the amount of slip caused by the icy conditions, and may have provided enough friction on the road surface to prevent the logging truck from losing control.
