Deceased was struck a glancing blow to the chest and abdomen by a lift of plywood which had dislodged from a forklift truck, unloading a trailer truck.

While bleeding off CO2 from a tractor trailer portable vessel, a fitting failed causing a steel pipe to swing around and strike a worker.
