New versions of forms now available
Richmond, B.C., September 22, 2008 — WorkSafeBC is simplifying claim reporting in 2009. Revised versions of the following claim-related forms are now available:
- Application for hearing loss resulting from exposure to long-term occupational noise (Form 4)
- Application for compensation and report of injury or occupational disease (Form 6)
- Worker's report of injury or occupational disease to employer (Form 6A)
- Application of worker asbestos-related disease (Form 6AB)
- Independent operator's application for compensation and report of injury or occupational disease (Form 6/7)
- Employer's report of injury or occupational disease (Form 7)
- Employer's report of injury or occupational disease — Fishing (Form 7F)
- Hearing loss and employment questionnaire (Form 25W40)
Changes include:
- Separate contact information fields
- Clearer questions grouped under headings
- Simpler wording
- More information gathered early in claims process to reduce need for follow up
- Enhanced employment and wage-loss questions
- For forms 6, 6/7, 7, and 7F, first aid, medical, and return-to-work questions have been added
- Form 6A is now submitted directly to employers not to WorkSafeBC
- Form 7 includes additional fine-tuning since its release in January 2008
Many of these forms are PDF files and require Adobe Reader 6 or later to view. To update your version of Adobe Reader, go to the Forms page and select the Get Adobe Reader button.

If you're an employer, the quickest and easiest way to report a worker's injury or illness is online. Go to Report injury or illness online (Form 7).
If you're a worker reporting an injury, please call the Teleclaim Contact Centre at 1 888 WORKERS (1 888 967-5377), or #5377 for Telus, Rogers, and Bell mobility customers.
