When to submit a Form 8 or a Form 11

The basics:

  • See your patient as per clinical requirements;
  • Bill WorkSafeBC (the Workers' Compensation Board) through Teleplan; and document in your chart as per usual.

Also send the appropriate form to WorkSafeBC in one of the following ways:

  • EFORM via the Teleplan network, or
  • Paper form via FAX to 604-276-3195 or 1-888-922-3299.

Form 8 — submit if:

  1. You suspect your patient may be disabled beyond the date of injury;
  2. The patient's claim is for:
    a. A back injury;
    b. A hernia;
    c. A shoulder or knee strain/sprain; or
    d. An occupational disease
    (e.g., repetitive strain injury, cancer, work-related asthma, work-related dermatitis).
  3. If worker's injury does not meet any of the above criteria BUT WorkSafeBC requests a Form 8 (Bill fee item 19927)
  4. If a First Report should have been sent by criteria #1 and 2 being met, but was not, please fill out and submit a Form 8 to WorkSafeBC.


Form 11 - submit if:

  1. Your patient's condition has departed from your previously reported expected treatment plan and recovery (i.e., you are changing treatment or your patient is not recovering as anticipated);
  2. Your patient's disability duration is beyond usual recovery norms;
  3. Your patient is ready for return-to-work intervention;
  4. You need assistance from WorkSafeBC in any way; or
  5. You have not sent in a form in the past four weeks and your patient is still unable to be at work.
