Family physicians are required to report to WorkSafeBC (the Workers' Compensation Board) using two reports: the Physicians First Report (F8) (PDF 117kb) and the Physicians Progress Report (F11) (PDF 117kb). Both reports use the same form. You simply indicate at the top of the form, which report you are submitting.
Submit this report to WorkSafeBC if you believe there may be time loss beyond the day of the injury or if the claim is for a hernia, back problem, shoulder/knee strain or sprain, or occupational disease.
Submit this report if the worker's condition or treatment has changed since last report or if the worker is ready to return to work.
