Reporting

Physicians

Physicians seeing injured workers in emergency must report to WorkSafeBC using Physician's First Report (Form 8/11). Hospitals may invoice salaried physicians submitting Form 8. Electronic submission via Teleplan is required. Faxed forms (604 276-3195/toll-free 1 888 922-3299) will be reimbursed at a reduced rate.

The Physician's First Report (Form 8/11) must be submitted if:

  • You suspect your patient may be disabled beyond the date of injury
  • The patient's claim is for a back injury, hernia, shoulder or knee strain/sprain, or occupational disease (e.g. repetitive strain injury, cancer, work-related asthma, or work-related dermatitis)
  • Worker's injury doesn't meet criteria, but WorkSafeBC requests a Form 8/11

Operating room reports

The surgeon's operating room report and the anesthetic record must be submitted before payment will be processed. Surgical time indicated in the anesthetic record is mandatory to validate the rate of payment.

Medical imaging

All medical imaging reports (MRI, X-Ray, CT-Scan) must be submitted to WorkSafeBC before payment will be processed.

Out-patient physiotherapy

The Physiotherapy Initial Notification Form (83W5) is only required for Stream 1: Standard Treatment. Use Physiotherapy Report (268) to make the following reports (indicate type at top of form):

  • Physiotherapy Standard Treatment Report
  • Physiotherapy Exceptions to Standard Treatment Report
  • Physiotherapy Home Visits Report
  • Physiotherapy for CNS Disorders Treatment Report
  • Physiotherapy Requested Report
