How to contact us

You can make a complaint to the Fair Practices Office by using our Fair Practices form, submitting a complaint in writing, or calling us with your complaint. After you submit your complaint, the Fair Practices Office will contact you within one working day.

Fair Practices form

To make a complaint using the Fair Practices form:

  1. Open the Fair Practices form (PDF 74kb).
  2. Fill out the form electronically, using the spaces provided.
  3. Print a copy of form.
  4. Send it to the Fair Practices Office by fax or mail. Please do not e-mail the form.
