How to make a complaint

You can make a complaint to the Complaints Office by using our Complaint form, submitting a complaint in writing, or calling us with your complaint. After you submit your complaint, the Complaints Office will contact you within five working days.

Complaint form

To make a complaint using the Complaint form:

  1. Open the Complaint form (PDF 74kb).
  2. Fill out the form electronically, using the spaces provided.
  3. Print a copy of form.
  4. Send it to the Complaints Office by fax or mail. Please do not e-mail the form.
