A worker has the right to request a copy of their claim file at any time, outside of an appeal or legal proceeding. All requests for claim file disclosure must be in writing and cannot be taken over the telephone.
To request a claim file disclosure, complete and submit a Request for Disclosure – form 25M13 (PDF 96kb). The request can be mailed or faxed to:

Receiving the disclosure
Disclosures are sent by Express Post mail and require a signature upon delivery. Arrangements can be made in advance to pick up the package at any one of the province's WorkSafeBC offices. Please provide detailed instructions with your request if this is your preference.
Requests for other type of personal information
All of the claimant's personal information is usually in the WorkSafeBC claim file, but sometimes there are other WorkSafeBC records that contain personal information. To obtain these records send your written request (specifying your name, claim number, address and telephone number) to:
